Free Medical ID Jewelry for Dialysis and Transplant Patients
The National Kidney Foundation of Maryland offers free medical ID jewelry (stainless-steel bracelet or necklace) for all patients. The social worker at any dialysis or transplant center can assist in obtaining the medical jewelry by completing a three-part application, including the following information along with the patient's name, address and unit.
Choice of: Bracelet or necklace
Front of Jewelry: Hemodialysis or Peritoneal dialysis or Transplant
Listing of any allergies
Back of Jewelry: Patient's name
The jewelry company will forward the bracelet or necklace to the dialysis or transplant center for distribution to its patients.
